What makes AL-605 stand out from other drugs in this class is that it was specifically engineered to cross the blood-brain barrier, the protective layer that normally blocks most medicines from reaching the brain. The trial is currently focused on cancer patients, but AL-605 is among the first brain-penetrant CDK2 inhibitors to reach human trials, which is significant for the MEF2C community because CDK2 has emerged as a potential therapeutic target in preclinical MEF2C research.
Why should MEF2C families pay attention?
Research from the Medical University of South Carolina, published in the journal Cell Immunity, identified CDK2 as a key protein involved in the brain inflammation seen in MEF2C haploinsufficiency syndrome. The study showed that inhibiting CDK2 could potentially reduce this inflammation, which is thought to contribute to some of the neurological symptoms associated with the condition. If AL-605 demonstrates a good safety profile and confirms brain penetration in humans in its current oncology trials, it could become a candidate for repurposing — a process where a drug approved for one condition is investigated for use in another. This is not a guarantee, and there are still significant hurdles ahead, but it represents a concrete step forward on a pathway that's been mapped out by researchers.
The same day, Alaw also announced a second drug, AL-433, a brain-penetrant CDK4 inhibitor heading toward first-in-human trials later this year. Separately, another CDK2 inhibitor, AVZO-021 by Avenzo Therapeutics, presented encouraging Phase 1/2 data at the ASCO conference in early June, showing favourable tolerability in patients. Having multiple drugs targeting the same pathway is encouraging — it means the scientific community is actively exploring this area, and if one programme encounters difficulties, others remain in play. The next significant update for AL-605 is expected in the second half of 2026, when interim trial data should become available.
